Information clause – information about the drug
According to Art. 13 sec. 1 and sec. 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data (hereinafter referred to as the GDPR), I inform that:
1. The administrator of your personal data is Przedsiębiorstwo Produkcji Farmaceutycznej HASCO-LEK S.A. with its registered office in Wrocław, at st. Żmigrodzka 242e.
2. The contact details of the Personal Data Protection Inspector are: iod@hasco-lek.pl
3. Personal data will be processed in order to answer questions sent to the e-mail address or via the drug information hotline. The legal basis for processing is Art. 6 sec. 1 lit. a), GDPR – consent of the data subject. The data will be disclosed only to entities servicing the administrator or its subcontractors authorized to process personal data and recipients authorized under the law.
4. Personal data will be processed during the exchange of correspondence, but not longer than 5 years from the date of receipt of the inquiry.
5. The person whose data is processed has the right to: request the Administrator to access their personal data, rectify them, delete or limit the processing of personal data, object to processing, transfer data, lodge a complaint to the supervisory body – the President of the Office for Personal Data Protection when the processing of personal data violates the provisions of the GDPR.
6. The rights indicated in point 5 can be exercised by sending correspondence to the Administrator’s address given in point 1, or the IOD e-mail address provided in point 2 2. The Administrator will exercise the indicated rights, if there are no circumstances entitling the Administrator to process them, of which the data subject will be informed.
7. Personal data will not be transferred to third countries and are not subject to automated decision-making, including profiling.
8. Providing personal data is voluntary, but necessary to achieve the purpose of processing.
Information clause regarding the processing of personal data in connection with the provision of assistance from the Guaranteed Employee Benefits Fund to protect jobs following the occurrence of COVID-19
According to Art. 14 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, I provide the following information below:
IDENTITY OF THE ADMINISTRATOR | The administrator of personal data is the voivodeship employment office to which the application for benefits for the protection of jobs was submitted in connection with the occurrence of COVID-19. |
CONTACT DETAILS OF THE ADMINISTRATOR | The Data Administrator can be contacted via the e-mail address provided on the website of the office to which the application was submitted or in writing to the address of the Administrator’s registered office. |
CONTACT DETAILS OF THE DATA PROTECTION OFFICER | The Data Protection Officer can be contacted in all matters regarding the processing of personal data, in particular in the scope of exercising the rights related to their processing, via the inspector’s e-mail address provided on the office’s website or in writing to the address of the administrator’s seat. |
SOURCE OF ORIGIN OF PERSONAL DATA | An entrepreneur who applied for benefits for the protection of workplaces. |
CATEGORIES OF PERSONAL DATA | Name and surname, series and the number of the identity document, PESEL number, amount of remuneration, and working time. |
PROCESSING PURPOSES AND LEGAL BASIS | Your data will be processed in order to consider the application sent to the voivodeship employment office for benefits for the protection of jobs in connection with the occurrence of COVID-19. The legal basis for data processing is Art. 15g of the Act of March 2, 2020, on special solutions related to the prevention, counteraction, and combating of COVID-19, other infectious diseases, and crisis situations caused by them and art. 6 sec. 1 lit. e GDPR. |
DATA RECIPIENTS | Your personal data may be transferred to other entities on the basis of applicable law (e.g. the Police, enforcement authorities, Social Insurance Institutions, and tax offices). |
DATA STORAGE PERIOD | Your data will be processed for the period of settling the matter, starting from the date of submission of the application to the date of settlement by the employer of the funds received from the Fund or until the date of completion of the inspection in terms of compliance with the provisions of the contract for the payment of benefits, spending the funds of the Fund for the payment of benefits for the protection of jobs in accordance with its purpose, proper documentation, and settlement of received and spent funds from the Fund for the payment of benefits for the protection of jobs. In addition, your data will be processed during the period provided for the archiving of documents entering the national archival resource. |
RIGHTS OF SUBJECTS OF DATA PROCESSING | You have the right to access your data and the right to rectify them, object, or delete them after the expiry of the indicated periods or limit their processing. |
RIGHT TO LODGE A COMPLAINT TO THE SUPERVISORY AUTHORITY | You also have the right to lodge a complaint with the supervisory body dealing with the protection of personal data in the Member State of your habitual residence, place of work, or place of alleged infringement, which is the President of the Personal Data Protection Office (PUODO). INFORMATION ON OBLIGATION TO PROVIDE DATA OR ABSENCE OF SUCH OBLIGATION | Providing personal data is voluntary, however, their processing is a condition for considering the application for benefits and its implementation. |
